The most common reason we see for this is that the EIN has been entered incorrectly in your Club's settings. Here's how to fix it:
1. In myICLUB.com , go to Accounting > Utilities > Update Club Settings
2. Edit your EIN so that there's a dash between the second and third digits, so it fits the pattern:
3. Save the change
Now when you regenerate the tax forms, your club's EIN should print correctly.
